Part One — Whole Person Profile

An internal tool allowing providers a comprehensive view of patients, including employer benefits through a patient’s whole person profile. This KPI project is part one of a larger KPI effort. Scroll down for part two.

Provider Connect — Increase Health Cloud utilization by Providers through a dedicated portal for their patients through enhanced support tooling.

Giving the Provider a complete whole person view along with a patients employer health benefits helps Providers deliver timely care when patients need it.

 

Part Two — Provider Messaging

An internal messaging platform allowing providers to connect to their patients through the Get Help feature in the patient app experience.

Bridging the communication gap providers once had between their patients with this new messaging tool that also increased Health Cloud utilization by 20%.

 

Get Help (Care Team)

A frictionless way for connecting patients with providers

Get Help (Care Team) addition for patients in their patient app experience.

Project Duration: 2024


The net benefit:

Patients can easily message their care team after completing one in-person appointment establishing the Patient / Provider relationship. Increased appointment scheduling, increased appointment follow-through, and increased messaging utilization by 18%.
